Quantum offers a range of specialized care programs designed to help patients recover, manage chronic conditions, and maintain independence in the comfort of their home. Our team creates customized care plans focused on safety, wellness, and quality of life.

We offer guidance and care coordination for safe transitions from hospital to home, including helping individuals with COPD, CHF, and diabetes, as well as offering programs for safety and independence, medication management, infusion therapy, wound care, joint replacements, and functional mobility.

HEALTHY HEART CARDIAC CARE PROGRAM


Heart failure is when the heart does not pump as well as it should. The heart is weak and gradually worsens over time. Some parts of the body may not get enough blood flow and oxygen. Systolic Failure is when the heart is not able to pump blood out. Diastolic Failure is when the heart cannot relax properly so the heart does not fill with blood as it should. 

Treatment includes:

  • ACE Inhibitors / Angiotensin II Receptor Blockers to open the arteries and lower blood pressure
  • Beta Blockers to block the effects of chemicals that make the heart work too hard
  • Digitalis, which makes the heart beat stronger and more regularly
  • Diuretics to help your body get rid of excess fluid, making it easier for your heart to pump
  • Vasodilators to open your arteries and make pumping easier
  • Aldosterone Blockers to prevent the effect of a hormone that may cause fluid buildup and high blood pressure

TAKE CHARGE DIABETIC CARE PROGRAM


There is no cure for diabetes, but it can be effectively managed by eating correctly, checking and recording blood glucose, solving blood glucose problems, reducing health risks, exercising and being active, and taking medications correctly. 

We provide assessments, monitoring, and home health diabetes teaching to help improve your quality of life and prevent complications. Depending on your needs and doctors’ orders, you may receive different types of care from various members of our interdisciplinary team.

IV INFUSTION THERAPY


Our home infusion therapy is a specialized service that needs the expertise of a highly trained infusion clinician. We train our clinicians regularly in the various intricate procedures required to become specialized in the field. For many medically stable clients, home infusion therapy (IV) is a safe and comfortable alternative to receiving IV therapy in the hospital or an outpatient clinic. With our approach, clients often experience more positive results from their treatments, recover faster, and have a decreased risk of encountering certain complications, such as infections.

Our infusion clinicians will evaluate each patient’s condition to determine and recommend an appropriate plan of care, working closely with medical providers and the infusion pharmacist. We always recommend that the patient have a teachable caregiver in the case of any emergency or natural disaster.

SAFE LIVING FALL PREVENTION CARE PROGRAM


Our fall-prevention and recovery program addresses musculoskeletal issues, visual disturbances such as cataracts and glaucoma, chronic conditions like Parkinson’s disease, medications, home safety, and functional and cognitive impairments. By providing skilled nursing, physical therapy, occupational therapy, speech therapy and pathology, medical social work, and home health aides, we help patients reduce fall risk and remain safe in their homes.

STAY FLEXIBLE ORTHOPEDIC CARE PROGRAM


Our program concentrates on preventing falls and recovering after a fracture. Our team evaluates all aspects of a patient’s daily life to create an effective, personalized care plan to help patients achieve the highest level of functioning and independence as possible. We consider physical challenges like balance, flexibility, muscle strength, and gait, as well as identifying hazards in our patients’ homes. Then we put an individualized care plan into action to significantly reduce fall risk.

  • Musculoskeletal Issues: issues with muscles, joints, nerves, discs, blood vessels, and other parts of the musculoskeletal system

BREATHE EASY PULONARY CARE PROGRAM


Our “breathe easy” pulmonary care program specializes in the management of patients with a wide variety of acute and chronic pulmonary conditions. Our experienced team members provide individualized support to ensure that patients are able to best manage their conditions, reducing the need for ER visits and hospitalizations, as well as preventing disease exacerbations.

We follow specific wound care instructions given to us by wound care physicians. Our approach to treating patients’ wounds reduces complications, promotes healing, decreases the chance of infection, and keeps patients healing safely at home. We always recommend that the patient have a teachable caregiver in the case of any emergency or natural disaster.
