      Common Indicators of End-Stage Disease

      A patient may exhibit one or more of the following core & disease indicators. Patients may be regarded as terminal within six months if the disease follows its normal course. 

      RENAL DISEASE

      • Creatine Clearance < 19cc
      • Diabetes < 15cc/min
      • No Dialysis
      • No Renal Transplant Planned
      • Uremia
      • Oliguria < 400cc/24 hours
      • Hyperkalemia > 7.0mEq/L

      DeMENTIA & ALZHEIMER’S

      • Unable to Walk
      • Speech Limited to ≦ 6 Words
      • Unable to Dress Without Assistance
      • Unable to Bathe Without Assistance
      • Complications Such As:
      • 10% Weight Loss in 6 Months
      • Albumin < 2.5g/dl

      LIVER DISEASE

      • No Liver Transplant Planned
      • INR > 1.5
      • Serum Albumin < 2.5g/dl
      • Ascites Despite Diuretics
      • Peritonitis
      • Hepatorenal Syndrome
      • Hepatic Encephalopathy

      HEART DISEASE

      • NYHA Class IV
      • Discomfort With Physical Activity
      • Symptomatic Despite:
      • Arrhythmia Resistant to Treatment

      PULMONARY DISEASE: COPD

      • Recurrent Pulmonary Infection
